Scheduling Appointment: Appointments can be scheduled via phone or email. If you choose to book an appointment through our website, please provide your name, phone number, and desired service(s) at least 24 hours in advance to ensure availability. A member of our team will promptly contact you to confirm your appointment. For STAT draws, please call us at 1-833-297-4624.

Appointment Confirmation: When scheduling your appointment, kindly email or fax us your doctor's requisition form, insurance card, and ID. This ensures that we have all the necessary documents prepared for your appointment. All appointments will be confirmed via email or phone 24 hours in advance unless booking an appointment for the same day. Please ensure your contact details are up to date.

Cancellations and Rescheduling: To better accommodate our patients we ask that any cancellations or rescheduling requests be made at least 24 hours before the scheduled appointment. Please note, refunds will not be issued for no-shows or cancellations made less than 24 hours before the appointment time.

Refunds: We understand that circumstances can change, and we prioritize clarity and efficiency in our refund process. Refunds will be promptly processed within 48 hours

upon cancellation of your appointment. To qualify for a refund, please ensure cancellations are made at least 24 hours in advance of your scheduled appointment. Please note, refunds will not be issued for no-shows or cancellations made less than 24 hours before the appointment time.

Payment: Your payment can be made online upon receipt of your invoice, which is sent via email after your appointment is scheduled. Please ensure the invoice is settled before your appointment. Payment must be completed prior to receiving any services.

Insurance: As a private care provider, we do not accept health insurance

and are unable to provide the detailed documentation, summaries, or

receipts needed for health insurance reimbursement. While our mobile phlebotomy services are not covered by insurance, we kindly request that you provide your insurance information for billing purposes related to any testing conducted by the lab. If you receive a bill from the lab, please reach out to them directly for assistance.

Privacy and Confidentiality: All personal and medical information provided by clients will be kept confidential and used exclusively for the purpose of delivering the phlebotomy service. Our practices adhere to all applicable privacy laws and regulations including the Health Insurance Portability and Accountability Act (HIPAA) guidelines. We ensure that your information is protected and handled with the highest level of security and discretion.

Liability: While we strive to deliver professional and safe services, we are not responsible for any adverse reactions or complications that may occur during or after the blood draw procedure. Clients are encouraged to inform our staff of any relevant medical conditions or concerns prior to the appointment for optimal safety and care.

Service Scope: Our mobile phlebotomy service offers professional blood draw services at your preferred location. Our certified and trained phlebotomists are dedicated to providing the highest standard of care. We are committed to delivering a seamless service that is both efficient and accommodating to your individual needs.

Client Responsibilities: Clients are responsible for providing a safe and suitable environment for the phlebotomist. Please inform us of any medical conditions or special requirements when booking your appointment. For a safe and sanitary blood draw, please secure any pets before your scheduled appointment.

Changes to Terms and Conditions: We reserve the right to modify or update these
terms and conditions at any time.
